50 year old female diagnosed with covid 19 pneumonia

50 year old female came to the opd with the chief complains of fever since 8 days and was tested positive for covid-19 3 days back.

HOPI:-

patient was apparently asymptomatic 8 days back then she developed fever. she had no history of shortness of breath, cough, fatigue, loss of smell or taste. she was tested positive for covid-19 3 days back and was admitted into the hospital. after she got admitted into the hospital she presented with complaints of oxygen saturations fluctuating between 86%-92%.

no h/o of DM, HTN, asthma, CAD,TB or any surgeries.

diet-mixed, appetite -normal, B&B- regular, sleep- adequate.

no h/o of tobacco or alcohol and no h/o of allergies.


Physical examination:-

no signs of pallor, icterus , cyanosis, clubbing, koilonychia or lymphadenopathy.

temp- 98.4F, pulse rate- 101bpm, RR- 20cpm, BP- 110/70mmhg.


CVS examination:-

S1 S2 heard

no murmers heard


CNS examination:-

patient was conscious and alert, oriented to time, place and person

speech - normal 

no signs of meningeal irritation

sensory and motor system are normal

Gait - normal

reflexes- intact


Respiratory examination:-

no h/o of dyspnea, wheezing 

position of trachea- central.


PROVISIONAL DIAGNOSIS:- COVID-19 pneumonia


INVESTIGATIONS:- 

CBP, LDH, LFT, RFT, PLBS, HbA1c, urine for ketones, serum electrolytes, CUE,ABG


















TREATMENT GIVEN:

1) head end elevation

2)O2 supplementation if spo2< 95%

3)T. DOLO 650MG

4)T. PANTOP 40MG

5)T.LIMCEE

6)Nebulization DUOLIN and BUDECORT

7)INJ HAI 6ML/HR INFUSION

8)T. OSELTAMAVIR

9)INJ.CLEXANE

10)SYP.CREMAFFIN

11)GRBS monitoring 2hrly

12)BP,PR,SPo2 monitoring

13)BETHADINE mouth gargling diluted with water 4-5 times/day
